I noticed a recurring trend in shooters that have grenade launchers, namely: why are the ladder-like sights they have so useless? They don't even remotely show where the grenade is actually going to fire, it doesn't really represent any arc the grenade travels in, etc. Why not just put a simple crosshair and be done with it?

I think they're supposed to show you the angle at which it will fire, then you're expected to calculate yourself how it will fly based on the apparent launching force and weight of the projectile.

They use the ladder like sights because that's what many real life grenade launchers use. If I recall to use them you put the appropriate ladder rung (depending on the range) over the tip of your barrel in order to get the right angle for the grenade to land at that range. That usually doesn't translate very well into video games where the average range of engagement is 100m or less and they don't set the viewpoint up to really use the ladder sight the way it was intended.
