It is random, in that the heart picks secrets randomly for generic NP Cs based on the type of npc your pointing at, but it feels like there's no secret that aren't about how whoever your aiming at isn't human scum.
I've had a few people who turned out to be nice people, but they were vastly outnumbered by people who were secretly the worst people ever.
One wonders how Karnaca even functions.

By the sounds of it, one would think that a Low-Chaos Emily would just give up and go for the whole "Kill Em All" approach. I mean, she's trying to better the lives of her people, but seriously, there comes a point when you realise that do these people WANT a nice, peaceful, governed life?

I am not even joking when I say this but when the Black Market salesman is the best person in town then you know something is wrong with this town.
The heart literally said that his worst secret was Holding her hand as she died which is not bad per say just depressing.
I actually saw an youtuber do that. Literally just snap and go on a killing spree after hearing a ridiculous amount of horrible secrets.

That's absolutely terrible. But it doesn't mean that a super majority of the population is a murderer (or even a criminal).
If something like 80% of your population is committing aggravated assault or worse...you aren't going to have a population for very long. That isn't sustainable.

I still haven't played Dishonored 2 yet, but based on the first game: Remember that while the Heart doesn't lie, it's also very cynical. In the first game, basically the only person it didn't have something terrible to say about was Emily, for obvious reasons.
I the first game it had more "This person is soldiering through adversity" kind of deal "Or that person has to do hard things to make good of a bad situation". Here it's far more gratuitous.
Mind you, Dishonored 1 had less lines in general for the heart And you can cycle through all of them by repeatedly pointing at the same person. Here it gives ONE secret per person, and it has lists for guards, for overseers, for men and women. There's far more secrets and it takes longer to see them all.
Some exerps of non negatives one from the first game:
- "Work is never done for the lowly servant."
- "Her servant wages will not be enough to fix her father's debts."
- "Only after a long day of work will he return to his home and family."
- "She starves so that her children can have bread."
- "That work was the best he could do. And still they weren't satisfied."
Dishonored one also had far more generic statements that weren't about that particular person, like #1. So I guess if you remove those, you do end up with a higher "This person is an asshole" ratio.

It could be possible that the Heart is possibly bugged and giving a lot more negative secrets about the folks of Karnaca then intended (I hear that it's suppose to be 50/50 but from what I have seen it's certainly not that) or maybe the Outsider changed how it worked a bit just to see how your reaction to all these fabricated nasty secrets might be.
Either that or Karnaca is way worse off then Dunwall and that reflects in the kind of people it attracts.

The funny part is that he was genuinely surprised that the citizens of Karnaca seemed so nice at first. And that was destroyed by most of the comments the heart made. Out of all of them I only counted three citizens that were decent folks and one of them was the Black Market Salesman.

- Phenom II processors are currently not supported
- Adaptive Resolution can get stuck at -1 when restoring video settings
- Changing the V-Sync option will allow the Adaptive Resolution slider to move again
