  • Game-Breaker: The various Touhou-related items run between useful, gimmicky, to snapping the game's difficulty like a twig. For example:
    • Fan of the Dead is essentially a Stone Wall multi-life system, without the drawbacks of most extra lives in Isaac. It trades every individual hit-point of health Isaac has for lives, at the cost of becoming a One-Hit-Point Wonder, which becomes moot due to the aforementioned lives. Even after picking up the item, you can still stock up on lives with health upgrades and soul heart drops, with this trait making it better than a normal health system due to it overshadowing the +20 cap. Even Vicious Cycle (a passive that gives you a constantly active Damocles, even after dying and reviving) becomes a minor annoyance with this multi-life system. Unsurprisingly, Lunatic Mode nerfs this item by limiting your max amount of lives with this item to 20.
    • The Leaf Shield. A shield of leaves that double as an attack alongside your tears. As long as you don't shoot, it'll protect you from all projectiles, with even Bullet Hell bosses such as Hush and Doremy being hard-countered by the item.
    • Wild Fury, a six charge active item that buffs your stats crazy high for ten seconds. That alone is strong for an active item, but it truly becomes powerful with this one aspect: every killed enemy, including those spawned by other enemies and bosses, gives a small but permanent all stats up towards your base stats. Boss Rush can be easily overcome with this item alone with the massive temporary buff that's constantly recharged, with the prize being enough stats up to bulldoze through most of the run.
    • There's also the Rune Sword, a six charge active item. When picked up, it spawns a Rune or Soul Stone. When used, it has one of two effects: If you aren't holding a vanilla/Reverie rune or soul stone in your inventory, it will spawn a Rune or Soul Stone. If you are holding a vanilla/Reverie rune or soul stone though, using the Rune Sword will absorb the Rune into the Rune Sword, granting powerful passive effects while holding the Rune Sword. Rather fittingly, Lunatic Mode nerfs this item by only allowing six runes to be inserted into the Rune Sword, but this still allows for a degree of game-break.
    • Benediction, a 12-charge active item. This item can be used at any number of charges, and grants you an angel pool item for the floor dependant on the number of charges. In particular, some extremely strong items can be acquired through this effect. You can bolster your damage through the 3, 5, 6, 9, 10 and 11 charge items (Angelic Prism, Trisagion, Godhead, Revelation and Sacred Heart, respectively), gain greater defense through the 1 and 7 charge drops (Hallowed Ground and Holy Mantle, respectively), or if you're truly dedicated to trying to break the game as hard as possible, you can delay visiting an item room until you've gathered 12 charges, which will give you Sacred Orb.
    • The Gap, a 4-charge active item that places a map of the floor on the ground. You may then walk over any displayed room and use the Gap again regardless of charges to warp to that room. The Gap will show all normal rooms on the floor, as well as the Secret, Super Secret and Ultra Secret rooms, essentially guaranteeing additional items per floor, which can also be extremely valuable damage boosters like the Dark Ribbon or Pure Fury. (or even Game Breakers like the aforementioned Wild Fury).
    • Lost Memory, essentially Reverie's own version of Death Certificate. On use, it takes you to a library, where you can take any items pertaining to one Touhou character, which of course includes the examples mentioned above. Notably, if the Touhou character has multiple items based on them, you can take all of those items.
    • And finally, there's the D2147483647, a "dice" item that mimics nearly every other active item in the gameincluding ones from other modsnote . There are only two downsides: It takes one additional charge to use, and it destroys itself upon mimicking itself or a one-time use item, though the latter case can even be exploited in certain situationsnote . Even with those downsides, the D2147483647 can singlehandedly earn you a free win.
  • That One Sidequest: The Photo Exam custom challenge. You start off on the Hush floor equipped with the Tengu Camera in the pocket item, an item that gives you collectibles and items based on your score earned by taking pictures of both projectiles and enemies; plus four Death Certificate items available to grab any item in the game, including the unbalanced Touhou-related items. The problems arise from the gimmick: Hush is invincible, they cannot take damage from your normal attacks, or even damage by other means. The only way to beat Hush, and by extension the challenge, is by getting the Death Certificate from the Tenga Camera via maximum score of 100, but as you're required to draw the aim of the projectiles / lure enemies in a way to score the maximum number of points along with having to avoid all that, and you have a frustrating challenge.
