• Continuity Lock-Out: Only a Lonely Heart relies very heavily on the reader having read and understood Sangue Serenissima and, to a lesser extent, The Englishman. Anyone who hasn't read it will have no idea who Seers are, who Eliphas Coyte was, or who Eliza Cortly is and why her disappearance has taken such a heavy toll on Hardestadt's emotions.
• Nightmare Fuel: The Defiler and his very nature invites this. The basket of bobbing heads is only the start, with the horrific degeneration of the Duvalier residence and Y'golonac's ensuing actions only raising it further.
• Paranoia Fuel: Y'golonac knows every defilement, every violation, every rape, whether or not the person who's even being violated realizes. He compounds it with one simple sentence when Hardestadt refutes his touch and states he's never been raped before: "are you sure about that?"
• Tear Jerker: While the exact details of what happened are still ambiguous (until The Wolves of War), whatever happened to Eliza has clearly made Hardestadt miserable, to the point where years over he still isn't over it. Seeing the man who days earlier slew a violent, child-killing vampire without breaking a sweat in such a state is sobering.
