  • Anti-Climax Boss: The penultimate and final stages in the Arcade version recycle Heavy-Metal Tony and Big Mamma (respectively) as a Dual Boss. This is in contrast with the Genesis version, where Heavy-Metal Tony is only fought once and Big Mamma has a whole new moveset in the second fight.
  • Difficulty Spike: The American Genesis port's Stage 2. You've only got 6 health blocks max, and it's a long stage that has multiple stage hazards including limited visibility sections, random cannonballs rolling at you (including during a significant chunk of the boss fight), and oil slicks - of which ONLY the oil slicks are present in the Japanese version, mind you - and you're introduced to Harry and Hatch, who deal way more damage per hit than most of the other enemies. If you're playing on Normal (which doubles the enemies' power and HP) and you didn't have enough money from the previous stage to buy either the defense or strength power-up, you're basically screwed from the start. Beyond this point, you'll be powered-up enough to where nothing's this hard ever again.
  • Disappointing Last Level: The Arcade version's final stage is a somewhat short level with a western theme that's rather incongruitous with the rest of the game's urban setting. The Final Boss is just two of a previous boss, which was already done in the preceding level. The game doesn't even have a proper ending after that.
