The film is set in an alternate future where Blofeld's plot in You Only Live Twice was successful, and America and Russia launched a nuclear war. This turned Earth into a post-apocalyptic dystopia, one where Blofeld could seize power. Over time, SPECTRE morphed into The Omega, bringing us to the present.
