Haruhi is a girl who is assumed to be male by most of the school. Basically, we have a trap and a reverse-trap who look identical. They are probably family. Maybe cousins.

* Well, their plays have male roles because the Zuka Club is a parody of the Creator/TakarazukaRevue, but this is still pretty plausible. Some of the males are as pretty or prettier than Benio and the only really 'feminine' characteristic that sets her apart from the actual males are her longer lashes and fuller lips, which could be accomplished with makeup. It's also said that her mother is a St. Lobelia alumni and a famous Zuka Club President and that Benio wants to follow in her footsteps; if he's male, he might have such admiration for his mother (and likely grew up in a female-dominated environment that echoes the school's/club's beliefs) that living as a female is part of attempting to live up to her example.

WMG Kyoya gender theory

Added DiffLines:

[[WMG: Kyoya does not identify as strictly male.]]
Kyoya does not appear to experience any discomfort in the anime nor the manga when referred to using feminine terminology, i.e. being the 'mother' of the group. Kyoya is also the most natural-looking host during the scene where they masquerade as Haruhi's 'big sisters,' signifying that Kyoya may have experience with makeup and feminine attire. This troper theorizes that Kyoya isn't uncomfortable with identifying as nonbinary/female/whatever it may be, but manages to go against such in presentation for the sake of the Ootori business's reputation. It is likely that Tamaki knows as well, as the two are close and Tamaki doesn't seem to regard Kyoya as a masculine entity.

Tamaki was a shy boy who lived with his dad and grandmother. He showed obvious signs of mental illness, but it was ignored because they were busy with work. When he was about to start high school, his dad died and his grandmother booted him off to live with his mom in France. His mother placed him in a academy, Ouran High, famous for lots of foreign students. He was soon put in the 'special' class, with Kyouya, Hunny, Mori, Hikaru, and Kaoru. He gets bored, so (without his friend's permission or knowledge) begins writing about them as a dating service. However, he made them more idealized, himself especially. Then their teacher gets a new TA, Haruhi, and he fell hard for her. But he was too shy to confess, so he put her in the story and had them fall in love there. Eventually, he gets so wrapped up in his story he can't tell the differecne between his friends and the modified versions of them in the story. They grow concerned and stick him in counseling, and he gets a little better. Eventually, when he's grown up, he finds his old story, and makes a manga out of it under a pen name.
