Karlach swore revenge against Enver Gortash for selling her to Zariel a decade ago, which forced her into the archdevil's service as a soldier in the Blood War as well as led to her heart being replaced with an infernal engine that would slowly kill her the moment she leaves Avernus. But once Gortash has been killed in Act 3, Karlach would soon lament that his death doesn't change the fact that she's still doomed to suffer a fiery death lest she returns to Avernus only to be hunted down by Zariel's forces.
