When it's not being hilarious, The Midnight Gang is being harrowing.
- Sally as a whole. She's weak due to her illness, which is heavily implied to be cancer, and people aren't even sure she'll live to grow up. In fact, her dream is to live a life, which, considering that the other dreams were implausible, is not very reassuring.
- When Tom lies that he didn't promise Sally anything, he feels so guilty that he thinks a part of him is dying.
- Tootsie the dinner lady pretends that she provides everything, even though she doesn't, just so the patients will temporarily forget what a sorry state the hospital is in.
- When Amber says that Sally can't join the gang because she'd slow them down, she cries a Single Tear.
- Nelly is delusional and thinks her father is still alive.
- Tom being wracked with guilt for indirectly getting the porter fired.
- The reveal that the porter has lived at the hospital all his life, being operated on repeatedly just to make him less ugly, and whenever he went out in public, people would be mean to him for his looks.
- Professor Pigeon, the porter's pet pigeon, also had a bad childhood: her mother kicked her out of the nest as soon as she hatched.
- The reveal that one of the early members of the Midnight Gang, Rosie, died because her heart stopped beating.
