Having created unforgettable bonds through their school career, the group of friends attend graduation, say goodbye to high school and look to their future.

Justin has been in hospital since his collapse at the school prom. The doctor is having a conversation with Clay and his parents, revealing that Justin had contracted HIV-1 which has developed into AIDS. She also tells them that he's suffering with meningitis and pneumonia. When they ask how this could have happened to him, the doctor states that HIV can be passed on via intravenous drug use and unsafe sex work. Clay is surprised as Justin had never mentioned he had partaken in sex work in the past.

Clay visits Justin at his bed and struggles to ask him about what the doctor had said earlier on. He's interrupted by Charlie and Alex before he can get his words out. Clay later discusses everything with Jessica. She tells him that she already knew about it and says that Justin may have been ashamed to tell Clay despite knowing Clay wouldn't judge him for it.

Meanwhile, Winston meets up with Tyler to make him aware that he has enough evidence to bring the group down regarding Bryce's murder however he will leave Tyler out of it if he wants. Tyler tells him that he's got the wrong person and if he does anything to his friends, the same thing will happen to Tyler anyway.

Zach struggles to enter the hospital and stands outside the doors, staring up. Charlie asks him what's wrong and Zach explains that his dad died in that hospital, hence why he can't bring himself to go in. Charlie says that his mother was in the same hospital before she died but he's still visiting Justin however Zach can't force himself to go in.

Winston meets with Alex who admits that it was him who killed Bryce. Winston doesn't believe him however Alex tells him to take the information and do what he wants with it.

At the hospital, Justin's health has deteriorated and he's now on ventilators. When Clay realises that Justin is terminal, he breaks down and leaves. He finds himself at the sheriff's station where he screams that he has a gun, causing panic and everyone to hide. Sheriff Diaz calmly talks Clay down, with Clay eventually showing that he doesn't have a gun and breaks down in the sheriff's arms.

During therapy, Clay admits that he didn't want to die that day at the station, he just wanted someone to hear him and see he was hurting. Sheriff Diaz didn't press charges which showed Clay that he actually cared about him.

Alex and Charlie find Zach, drinking whiskey alone. They tell him that he needs to get it together and visit Justin before it's too late. Zach claims that he's a lost cause however Alex tells him that he saved his life once and he's doing the same for him now. Zach finally gathers the courage to see Justin. When he gets to the room, Clay is already there. Zach hugs Clay tightly.

Tony arrives home and finds Caleb facetiming Tony's dad. His dad tells Tony that he should sell the garage and get himself to college. He says that the garage was never his dream - his only dream was to see Tony have a better life and a college scholarship will do that.

At the hospital, Jessica hallucinates Bryce who tells her that she finally wins. Jessica blames him for killing Justin. Everything that has happened started with him raping Hannah which lead to her suicide. She states that Bryce deserved to die.

Clay is called to see the Dean at school and is appointed the class speaker at the graduation ceremony. Clay states he's not sure if he can do it when Justin's dying in hospital.

Later in the day, Clay and his dad, Matt are talking in the hospital. Clay apologises for what happened at the sheriff's office but his dad says that he will always worry about him. Matt states that high school is definitely something that needs to be survived but there is joy to be had too.

At school, Zach meets up with Coach Kebra who offers him a coaching job. Zach admits that he took the coaching recommendation letter out of the coach's file because he didn't feel he could live up to the job. However, Kebra believes he can if he can get sober and graduate from school.

As his illness continues to progress, the doctors at the hospital take Justin off the ventilators. Everyone prepares to say their final goodbyes to Justin. Jessica is first to see him and immediately breaks down in tears upon seeing him. Justin tells her that she's so strong and Jessica says he is the best thing that ever happened to her. When Justin responds by telling her he ruined her life, Jessica disagrees and tells him that he taught her what love is like. Clay says goodbye to Justin with the two brothers sharing their love. Justin asks Clay to stay beside him and hold his hand. Justin finally passes away with the Jensens by his bedside.

The funeral from the beginning of the season is then revealed to be Justin's. Many people attend, most of which are students from the school. Alex tries to make a speech but starts to panic halfway through and is unable to finish.

After the funeral, Clay and Jessica stay at the church together whilst Alex, Zach, Charlie and Ani go to Monet's. When there, Alex thanks Ani for saving his life more than anyone else he knows. Winston shows up unannounced and asks Alex if he can speak to him outside. Winston asks him why he killed Bryce and if he was sorry for doing it. Alex explains that all he saw was anger when Bryce had hurt everyone he ever cared about but he's more sorry for what happened than he could ever show. Winston tells him that he's not going to say anything to anyone about it because even though he loved Monty, he also loved Alex, too.

Meanwhile, Tony decides to sell the auto garage and go to college. He asks Caleb what he's going to do if he ever gets lonely because going to college means he'll be moving away. Caleb reassures him that they're an eight hour drive away and they'll make it through the distance.

At the sheriff's station, Sheriff Diaz and Deputy Standall discuss Bryce's case. Diaz confirms that the case is now closed and that family is the most important thing in life, implying he found out it was Alex who killed Bryce.

Jessica and Diego get tested for HIV and receive negative results. Diego apologises to her for thinking she was involved in Bryce's murder. He asks her if he can ask her on a date again in a month's time to which Jessica replies, "Maybe".

Clay receives the package of Hannah's original tapes from Olivia Baker. She explains that they were given back to her by lawyers after the lawsuit trial and she believes they belong to him now.

During his therapy session, Clay has a breakdown and explains that he feels nothing matters now that Justin is dead. He's upset that Justin allowed himself to become sick and that it would have been easy to save him. Dr. Ellman points out that Justin may have felt that he wasn't worth saving at the time and maybe, Clay has those same feelings about himself. He confides in Clay that he was a troubled kid and contributes his success to a teacher who made him see how turning his life around would be a positive change. Clay ultimately decides to confide in Dr. Ellman properly and opens up about everything that happened since meeting Hannah Baker.

The graduation ceremony takes place with students and their families attending. Jessica gives an impassioned speech about love and Clay gives his own speech about surviving high school and finding the best in life. After graduation, the group gather in the gym with their parents, including other students like Courtney and Ryan. They all talk about what their plans are and what they'll be studying. Jessica and Alex are going to Berkeley together, Tyler is going to continue studying photography and Zach is taking up music. Ani reveals she is going to donate the fund Mrs. Walker gave to her to the H.O club. Jessica asks Estela to take her place as the H.O leader.

Clay can see hallucinations of Bryce and Justin sitting on benches next to him. He explains to Justin that he knows he's not seeing ghosts, he just imagines people being there. Justin talks to him about forgiving people and Clay thinks of Hannah, who appears on the other side of the gym. He stands up to make his way over to her when he's interrupted by a student called Heidi. She tells him that she's going to Brown and likes Alien Killer Robots, just like Clay. She asks if he wants to go to coffee to chat and Clay agrees.

Later that day, the group from the tapes gathers to bury Hannah's tapes in the same spot Clay listened to his own. Jessica has a final hallucination of Bryce, telling him that he didn't win and his time there is over. The group look over the town and agree to be there for each other if anything happens.

At home, Clay has an interaction with the Justin hallucination. He reads an essay Justin wrote that got him into college and realises it's about him. It states that Clay was a positive influence on him and that Justin considered him his brother. Clay tearfully asks him how he'll go on without him to which Justin replies, "You'll survive".

At Clay's last therapy session before college, he agrees to keep in touch with Dr. Ellman.

The show ends with Tony and Clay driving to college to embark on their future.
