J.D. has custody of Sam and has taken him to Disney, so Elliott and Turk are in charge of the interns during a full moon. They start the evening with a prank to keep people's egos in check. Soon, however, more serious matters emerge.

- Actually Pretty Funny: One of the interns reacts this way after Turk scares the group. He understandably screams, recovers, and starts laughing.
- Jumpscare: In-universe, Elliott has Turk hide in a Hazmat bin with a mouth full of fake blood. Then she tells a scary story about a patient who bit off his tongue that they couldn't reattach, so she put it in the bin. Later, they heard strange noises from the bin, and opened it carefully. Cue Turk rising and trying to speak with fake blood spurting.
- Not So Above It All: Even Denise freaks out with the prank that Elliot and Turk pull on the interns.
- Serious Business:
- Turk is mad that J.D. went to Disney without him because they promised to go on the Nemo rollercoaster together. Elliott tells him to focus because it's just them in the trenches tonight.
- Todd takes offense at people doing high-fives without him. And he says it without any of his usual innuendos.
- Written-In Absence: The network wanted to see if the show could happen without J.D. and tested it out in this episode.
