Given this show's propensity for twisted, over-the-top violence, expect quite a few graphic moments.
- Robin's death in the first episode. She's instantly pulverized after being run through by A-Train and you can even see bits of her spinal column in the mass of gore that falls onto the ground.
- That poor bastard who got down and dirty with Popclaw. Who could have guessed her pelvis was that powerful?
- Expect anyone who gets killed/injured by Eye Beams on this show to make you want to vomit.
- The gills on The Deep's torso are just... unsettling to look at. Especially when on one occasion, a woman is inserting her fingers into them while she's raping him. Which is actually the point, as The Deep has a lot of insecurity and trauma from feeling like a freak because of his gills.
- Black Noir prefers to kill people with his bare hands, leading to scenes like him ripping a terrorist's head in half at the mouth.
- Stillwell's successful Wife Husbandry of Homelander... until he kills her, of course.
- Homelander further pursues his fetishes, first drinking a bottle of (the late) Stillwell's breastmilk that he kept and then later forcing Doppelganger to live out his incestuously-themed fantasies. His interest in milk tends to perturb many viewers, especially the slow, animalistic way he laps it up.
- Butcher driving a boat directly into a whale in Season 2 Episode 3, disemboweling the creature but failing to completely go through it, meaning the Boys have to dig themselves out of its innards to escape the Seven.
- Kimiko ripping a man's face off in Season 2 Episode 5. (This was the Female's signature move in the comics, making its appearance on-screen a Mythology Gag.)
- Homelander and Stormfront having sex after he crushed a man's head, both of them making out with their faces caked in the man's blood.
- MM getting strangled by the, uh... "love sausage". But also really funny.
- One of the Compound V experiments develops corrosive vomit...without the Required Secondary Power of being immune to it himself.
- What Ryan's heat vision does to Stormfront. Of course, she absolutely deserved it, and even brought it on herself, but the results are horrifying nonetheless. Not only is she burned beyond recognition, but the blast completely destroys her previously damaged eye, and dismembers her, leaving mangled stumps with bone poking through. Not to mention that she's found laying in a small puddle of her own blood and melted tissue.
- The infamous sequence where an expy of the Atom shrinks down so he can enter his lover's penis and stimulate the nerves in his urethra. Then he sneezes, which causes him to revert back to his default size...
- Homelander getting a handjob from a dismembered and disfigured Stormfront.
- Homelander drinking raw, unpasteurized cow's milk
. - The flashback showing Black Noir's disfigurement. Mallory finds him with half of his face burned beyond recognition, with his eye being completely white and partially melted, just sitting on the ground, desperately reaching for his helmet while grunting in pain, as the attack left him unable to physically speak.
- The Deep being forced to eat Timothy the octopus. Not only is the act of eating a full-sized, living octopus on it's own nauseating, but Deep, who can communicate with marine life literally hears him begging for his life and praying before he's ultimately crushed to death by The Deep's teeth, as Ashley looks on in horror, and Homelander as well as his supposed wife look on with sadistic glee.
- Everything about the sexual debauchery and extreme gore that occurs during Herogasm. Everything.
