Why doesn't Hook just kill Peter while he's asleep, instead of poisoning his medicine? Obviously cause he wants him to suffer. Meaning this poison is meant to kill you veeery, veeery slowly. Think of how long Tinkerbell, who is many times smaller than Peter, managed to survive after drinking the poison and before she's healed by clapping. Peter would have been in agony for hours, probably.
