In the club, Baby Doll doesn't have much more control than she does in the real world, she's still awaiting a horrible fate just a few days away. Further, the Club scenario initiates while Sweet Pea is undergoing treatment. In the club, though, for Sweet Pea [[spoiler: who's used to Blue's abuses which Baby Doll is ignorant of at the start of the movie, the shift from abused mental patient to showgirl is a measure of control]], further she asserts that she should have the authority to call the mission off when she sees fit. Baby Doll is consciously in the asylum the whole movie except when she's undergoing treatment, at which point she goes straight into the war scenarios.

Recall at the beginning he was dressed in a strangely plain outfit, all white----it doesn't look like a doctor's uniform, but almost like hospital scrubs. As though his own clothes had been confiscated. Also, there's a deleted scene on the DVD which reveals that he is in fact a performer with the girls---but he doesn't dance, he sings. So if 'dancing' is the girls being raped, is Blue being raped orally by the men?

The Asylum is Reality, the Brothel is Fantasy Level I, and The Video Game missions are Fantasy Level II.
MPD is nowadays called DID: Dissociative Personality Disorder, where the victim of abuse escapes into his mind by creating Alters, to deal with the horrors of reality.
After the guilt of killing her sister, the girl we know as Babydoll created 4 aspects of her inner personalities, based on 4 girls she saw at the asylum.
She projected those insane asylum girls into Fantasy Level I, as parts of her.
A strong girl, a protective girl, a shy girl, and a practical girl.
In the real asylum she tried to escape, but realized that the real freedom is from the pains of Memory and guilt, so she sacrificed "body freedom" for "mind freedom", and allowed on of the girls of the asylum to escape instead.
We never see the girl escaping but we know about it from Dr. Gorski's dialogue with the Lobotomist.
The movies director said that no one died in the asylum. The 3 girls dying in the Fantasy Level is 3 alters inside the girl's mind.
With accepting the lobotomy as "true freedom" she allowed her main personality to die also, leaving only the Sweet Pea personality, trapped inside a catatonic body, but living freely inside the mind.. knowing that an actual girl escaped in reality and probably living happily in the inside world.
This twisted satisfaction is the reason behind the content smile on Babydoll's face after the lobotomy..
She is guilt free and memory-free, traveling on a bus on a long journey of inner healing, with a wise driver who is a character from her Fantasy Level II (whom the actual girl who escaped the asylum never knew)

That's why Baby Doll paralells the lobotomy doctor with the "High Roller". The "High Roller" would take her virginity in the bordello level, and if/when the lobotomy gets done, Blue would "rent" her, in an [[KillBill "I'm Buck"]]-style. That's also why Blue, a simple atendant in the asylum level, turns into a mafioso pimp.
