The story starts off with Gepetto's wife Marissia becoming pregnant and he had a dream that he would have a son and that he would teach his son everything about marionettes. But unfortunately, his wife ended up dying during the delivery and she gave birth to twins: Mia and Pinocchio. But then, Pinocchio dies shortly after the birth, while Mia survived and this causes Gepetto great pain as he wanted a son, not a daughter. Years later, Gepetto ended up building a marionette that represented his lost son Pinocchio and he treated the puppet like his real son. 30 years later, Mia, now an adult, finds out that Pinocchio is now alive and wants to kill her!

* LeakingCanOfEvil: Scarlet is so strong that even when she was still locked up in the underground lab, a TailorMadePrison meant to keep her sealed away, she still managed to influence the area around her to a lesser, yet still unnervingly powerful extent. All the paranormal activity Marion witnesses as she journeys through the facility is all her doing despite being imprisoned at the time.
* MotherOfAThousandYoung: Scarlet was made into one of these after being captured by greedy scientists. Imprisoned and kept in a weakened state, her tumors are farmed and implanted into others to create HalfHumanHybrids and monsters.
* MultipleEndings:
** [[ShaggyDogStory Ending 1]]: If Marion decides to get in the car, she gives up on her search. Unfortunately, she forgot that she can't drive, resulting in her crashing and dying immediately.
** [[AloneWithThePsycho Ending 2]]: If Marion doesn't give the heart to the ghost, she's caught by Helga and has her legs cut so she can't escape the mine anymore.
** [[GoldenEnding Ending 3]]: If Marion gave the heart to the ghost, she rescues her from Helga. She then escapes the mine alive, warning everyone to stay away from it. According to WordOfGod, this is the canon ending.
** [[CruelTwistEnding Ending 4]]: If Marion gave the heart to the ghost, but chooses to stay in the mine, she encounters Scarlet, who tells her she was trying to keep Marion away from the mine, but now it's too late. [[spoiler: She then decays and dies, infecting Marion, who mutates into a red-skinned monster. Horrified, she tries to kill herself, but instead her newfound powers go out of control, [[StableTimeLoop resulting in her travelling to the past and being found by the scientists, becoming Scarlet]].]]
* PosthumousCharacter:
** Marion's mother, who died at the start of the game.
** [[spoiler: Marion's father, who died in a mining accident. He was actually killed by one of the monsters from the underground lab.]]
* SpikesOfDoom: Particularly in the mine area, the floor may be made of retractable spikes that go off in intervals or are triggered by buttons. Fortunately they can't harm Marion, instead obstructing monsters.
* StableTimeLoop: [[spoiler: Marion continuously encounters strange creatures through the mine, birthed by a dangerous mutant named Scarlet. As it turns out, Scarlet is actually Marion herself, transformed into a mutant after being infected. Horrified, she tries to kill herself, but instead she's sent to 1981 where she's found and experimented by the scientists. Thus, Marion not only sets in motion the events of the main story, but also becomes her worst enemy and gets infected by herself. Whoa.]]
* TooDumbToLive: Yes, Marion. Go and release the woman in the containing chamber who's been there for 10 years in an abandoned lab filled with mutants. I'm sure nothing can go wrong.
** One of the scientists gave a chainsaw to one of the mutants because she asked so. He regrets it later, lampshading the trope.
* VillainTeleportation: One of the monster has the power to teleport during chases.
