* In ''VideoGame/TheSagittarian'', surviving the first chapter alone results in a second chapter where every choice gets the protagonist killed - he either brings the fight to the infected who end up overwhelming him, or he becomes deranged and is either killed by the biker, or kills him and is later gunned down by his gang.

** A later update added SCP-1499, which lets you enter an different dimension entirely inaccessible to SCP-096. While it's only a temporary protection, there is nothing stopping you from instantly putting SCP-1499 on whenever SCP-096 gets close, and removing it as soon as it despawns. In fact, some players managed to play beat the entire game with SEVERAL instances of SCP-096 active from the beginning of the game.

* ''VideoGame/SuperMarioMaker'' has an AntiFrustrationFeature deliberately designed to prevent this by making it mandatory for the designer of the level to complete it first before being allowed to upload it. Crafty players ''still'' found ways to make truly unbeatable levels:
** One tactic was to abuse how some blocks behaved slightly differently between updates: make and beat a level on version 1.0 and then upload it, and everyone else on newer versions cannot beat it.
** Another involves dumping an utter deluge of mushrooms on Mario and then forcing him to make a very particular jump. In the upload check ''all'' of these will be regular mushrooms and the jump will be possible, but in regular gameplay it's guaranteed at least one of those mushrooms will be the Weird Mushroom (which appears 1% of the time) -- the Weird Mushroom gives altered jump physics that render the jump impossible. Ironically the reason you don't get the Weird Mushroom during upload checks was to ''prevent'' making levels that were unwinnable unless you got the 1% Weird Mushroom drop.
** Another involves using Pink Coins and placing one in the main level and one in an unreachable subworld, so the unreachable one will be loaded during the upload check but ''not'' loaded during the actual game, and then having a "key check"[[note]]A design popular in pacifist levels where a player is only allowed to pass if they ''don't'' have a key[[/note]] somewhere between the two Pink Coins. The uploader will only get one coin and will be allowed to pass, while any player will get the ''only'' coin, and thus a key, and then be blocked by the key check.

* ''VideoGame/SystemShock'' requires that you be trying to lose, but does allow you to mess things up irrevocably. If you destroy the X-22 Isotope, you can't activate the station shields and blow up the mining laser. If you destroy ''all'' the Plastique, then you can't destroy the antennas and stop SHODAN's third plot. If you destroy the [[spoiler: isolinear chipset]], you can't hack SHODAN to win the game. These require that you do things that are counterintuitive, and the items in question are all resistant to damage, but they are notably ''not'' immune. The only real time that the game is completely unwinnable, however, is if you try to blow up the antennas without resetting the cyborg conversion chamber on the Engineering deck: [[spoiler: one of the antenna rooms is a trap: you'll blow up the antenna and also yourself, because SHODAN traps you with the explosion.]] Failing to reset the chamber means you die.

* In the SNES version of ''VideoGame/{{Shadowrun}}'', there's a chain of this, with the first one due to GiveMeYourInventoryItem with a consumable with only one use at the time. At the beginning of the game, if you used up the Slap Patch to heal yourself (perhaps after finding your first weapon, which is followed by enemies opening fire on you)? You have nothing to heal the shaman with, which is needed to unlock a topic. But even if you heal the shaman, the required topic is only gotten by "Talk"-ing to them, which is impossible if you don't do it at the first opportunity, as it's possible to end the interaction before that, and the shaman disappears once the interaction ends. The topic is only used two-thirds into the game, it's needed to enter an important location.
