Thomas Lupo commits suicide via overdose in a hotel with the help of an unseen partner. After his body is discovered, it's noted his watch and wedding band are missing. His wife notes he was distant in the previous weeks and tells Green and his temporary partner Joe Cormack to speak to Thomas' brother Cyrus, a former patrolman in the 2-7,
now working over seas for the NYPD Counter Terrorism division. Lupo is adamant his brother would not commit suicide, having defeated cancer five years prior. At his brother's home, he discovers the missing watch and ring in a lock box in the garage. Lupo uses Green's name to get Rodgers to test Thomas' body for cancer; sure enough it returned, and was stage four. He was killed by an injection of Potassium Chloride. Green discovers Lupo planned his affairs, mortgaging his business to pay off his mortgage for his wife and children. Lupo pushes for a formal investigation with him teaming with Green, which is turned down as it is against policy. Shortly after Lupo leaves Van Buren's office a call comes in; another death matching Thomas' has been discovered.

Rodgers notes both Thomas and the new victim, Larry Driscoll, were killed with the same cocktail of three drugs, which are administered in a sequence. It's the M.O. of "Dr Death" Dave Lingard, recently released after 10 years in prison for helping with assisted suicide. An advocate for the right to end one's life, he was convicted after admitting on television to helping with a suicide. Lupo is waiting outside Van Buren's office for news, and she shuts the door. She tells Green she is putting Lupo with him on this case; the second victim eliminates the conflict of interest; Green is skeptical but accepts. Lingard denies culpability, stating someone stole his formula but Green notes no one used it while he was in prison. Lingard claims he is strictly following his parole, not evening giving advice before going on a rant about "the tyrants" who make the laws against assisted suicide.

Following up a lead, the detectives find a nurse named Mila, who the detectives deduce is Lingard's daughter. She too is an advocate of assisted suicide, and admits to dispensing advice, but stops short of admitting to administering the injections. She claims the men committed suicide on their own and that video exists proving it. The footage is held by Bill Nolan, the journalist whose interview sent the senior Lingard to prison. Nolan gives the detectives the videos and they watch them with Van Buren and Rubirosa. Larry is seen on camera hooked up to the machine speaking to Nolan. He gives the reason for his death as being diagnosed with ALS, and not wanting to die a slow death. However, his medical file shows Type Four Spinal Muscular Atrophy, a non lethal condition. Rodgers says Larry was misdiagnosed a few weeks prior. Given immunity, a case would be hard to prosecute given the taped statements, Mila claims Nolan gave Larry a false medical report in order to convince him to go through with the suicide so Nolan could film it. When new ADA Michael Cutter gets Mila to confirm Larry would not commit suicide without the ALS diagnosis, he has Nolan arrested for second degree manslaughter.
