And, most specifically, [[spoiler:the Doctor breaks reality by stepping out of the TARDIS (the only place where causality is safe) because he couldn't bear to watch one version of Anji and Fitz being murdered, and tried to save them. This created two universes: one where the Doctor left the TARDIS, and one where he didn't, and they infinitely multiplied as Eight tried to fix the past via the TARDIS and only made things worse]].
