Sometime later, Regina has partially converted the operating room into a hive, where her "children", large carnivorous grubs that feast on human flesh, are housed in various cells. Taking advantage of this, she manages to unhypnotize Deborah by stuffing cotton balls in her ears, but is too late to save Carlos, who wanders into the operating room and has Regina kill him by stinging him in the forehead.
The two remaining teens proceed to make a run for it as Regina sends her makeshift drones after them. The hypnotized doctors nearly manage to apprehend and/or kill the duo, but they are able to fight them off. Deborah suddenly tells Trinice that doing so would destroy Regina's career, paying no mind to the fact that her idol is a murderous monster who killed one of her closest friends. A hysterical Trinice has been stuck to the wall of the hive as the hypnotized Deborah approaches her. Deborah mentions that while she genuinely feels bad about Trinice's predicament, she also mentions that Regina has (somehow) promised her "backstage passes for life" and has allowed her to act as a nanny for her "children". She picks up one of the grubs to show Trinice, and explains that she only has to remember to feed them, as well as the fact that Regina said they could start with her. Deborah approaches Trinice robotically, her former friend only able to scream in horror before the grub devours her face.
Before anymore can be said, Deborah's mom (Monica Louwerens) enters and determines that the teens appear hungry, offering to make dinner. When Deborah offers to just snack on leftovers, her mom refuses to let them eat a cold meal and gives them her wallet so they can order pizza. Getting back to the topic of Regina, Deborah is forced to admit that her mom went home because Regina was bringing in her own doctors and shutting off an entire floor of the hospital to keep out the paparazzi. She picks up one of the grubs to show Trinice, and explains that she only has to remember to feed them, as well as the fact that Regina said they could start with her. Deborah approaches Trinice robotically, her former friend only able to scream in horror before the grub devours her face.

Trinice (Olivia Hawthorne), Deborah (Hannah Kepple), and Carlos (Nico Gomez) are three teenage friends who are spending the night at Deborah's place. The trio are also huge fans of famous pop star Regina (Kaelynn Harris), and after drawing Trinice's attention away from one of Regina's music videos, feud among each other over who is the biggest fan, with Deborah claiming the title because she is the only member of the trio to have seen her live. While also bringing up the fact that Regina is nine months pregnant, Deborah warns Carlos to put out his cigarette, not wanting her mom to smell the tobacco. Despite the claims that Deborah's mom is at work, they hear her approaching and hurriedly try to air the room out. Deborah, while explaining that her mom's shift at the hospital got cancelled at the last minute, blurts out that Regina has gone into labor.
Before anymore can be said, Deborah's mom (Monica Louwerens) enters and determines that the teens appear hungry, offering to make dinner. When Deborah offers to just snack on leftovers, her mom refuses to let them eat a cold meal and gives them her wallet so they can order pizza. Getting back to the topic of Regina, Deborah is forced to admit that her mom went home because Regina was bringing in her own doctors and shutting off an entire floor of the hospital to keep out the paparazzi. While Carlos wants to take pictures of the baby and sell them for profit, Trinice still opts that the trio go to the hospital themselves to witness the birth, pressuring Deborah into accepting the offer by reminding her that she's Regina's number one fan. With that, the trio swipe Deborah's mom's employee key card and drive to the hospital.
Outside the hospital, Carlos manages to use his phone and Trinice's selfie stick to trick the outside camera so they can sneak in. Making it to the sealed off floor, the trio are spotted by a security guard (Bryce Anthony Shepperson) and told to leave. When the guard discovers Trinice carrying the stolen key card, the two engage in a struggle that ends with the guard being pushed down the stairs and possibly dying. Deciding not to stick around, the trio gain access to the sealed off floor, which strangely has all the lights turned off and a strange sound echoing throughout the halls. The elevator opens, prompting the teens to scatter. A surgeon wheeling a dead body on a gurney travels through the halls. Trinice, after accidentally knocking over a magazine rack and alerting the surgeon, discovers that they have mysterious glowing green eyes. Deborah suddenly reports to Trenice and Carlos that she has found Regina, drawing their attention away from the strange surgeon.
The three soon discover Regina in an operating room, being tended to by more green-eyed surgeons. One of the surgeons injects Regina with something that causes her great pain. Suddenly, antennae, segmented eyes, and insect legs burst forth from Regina's body, revealing her to be a giant, heavily pregnant hornet-like insect queen. Captivated by the scene, Carlos absentmindedly takes a picture, alerting Regina to their presence. She gives off a screeching, high-pitched frequency that breaks the window the teens were watching her from, which also happens to be what is hypnotizing the hospital staff. Deborah and Carlos are similarly hypnotized, but Trinice, who has her earbuds in, is immune. Taking advantage of this, she manages to unhypnotize Deborah by stuffing cotton balls in her ears, but is too late to save Carlos, who wanders into the operating room and has Regina kill him by stinging him in the forehead.
The two remaining teens proceed to make a run for it as Regina sends her makeshift drones after them. The hypnotized doctors nearly manage to apprehend and/or kill the duo, but they are able to fight them off. Inside the elevator, Trinice tells Deborah that they can use Carlos' picture to expose Regina as the monster she truly is. Deborah suddenly tells Trinice that doing so would destroy Regina's career, paying no mind to the fact that her idol is a murderous monster who killed one of her closest friends. Seeing that she and Trinice aren't on the same page, Deborah removes the cotton from her ears and willingly submits herself to the hypnosis.
