Easy. The Miracle, God, saw a young man begging for an agonizing punishment for some unknown sins, God grants his wish and punishes him. However, the church learns of this and creates a cult around the punished man, now known as the Twisted Father, and this is where things really go off the deep end.

This is the last straw for God, who transforms the pope and all that seemed to take his throne out of greed and false faith into monsters that plague the land, really this doesn't change much since all the Miracle did was expose the evil within the people's hearts.

A brotherhood of true holy men remain though, however they are soon massacred by the church leaving a single survivor, the Penitent One, who was raised back from the dead by the Miracle himself to atone for the sins of Cvstodia and somehow save the souls of his countrymen through faith alone.

The entire nation is indoctrinated into the cult, and the cult preaches of how forgiveness comes through penitence alone, flagellation (holy self harm) is one of these means. The entire nation now "atones" for their sins by hurting themselves rather than righting their wrongs and truly repenting, instead they confuse the agonies they inflict upon themselves, and eventually each other, as holy works.
