So far as the pageant goes, maybe it really is just so focused on physical appearance that typically no one without a slim, athletic build ever dares apply - they could have at least made someone involved in the "revolution" have a more average build to put the point across better though; chances are it would end up being Hannah, since Millie and El's respective physical appearances are too relevant to the plot to be changed.
