** "Then came a spear of light, piercing Hatred's heart" refers to Inarius killing Rathma, Lilith's favorite son. She may be a demon, but it's clear she loved Rathma dearly. His death also set the chain of events that led to Mephisto being let loose on the world, so "And he who was bound in chains was set free."

When it wasn't enough to get a response from Heaven, he broke down and walked back. While he was wrapped up in his turmoil, Lilith pulled herself off the spear and stabbed him in the heart with it and tearing his wings off with her bare hands, seemingly killing him. This fulfilled the prophecy because it was Inarius himself that was the "heart of hatred". Lilith is a hateful being, but her actions in this game were motivated at least partially by a twisted love for Sanctuary and her human offspring. Meanwhile, Inarius was motivated by his hate and disgust towards Lilith, humanity, and Sanctuary as a whole. Of the two, he was the more hateful in the plot.]]
