* MyGreatestFailure: Takumi considers failing to save Nanami at O-Front to be this. He's misled into believing briefly that she's alright but then realizes that she was tortured and had her arm chopped off because of what happened, and he blames himself for her getting involved in this because he wasn't strong enough to help her.
