As the trope name implies, this is a trope about appeasing a volcano god with human sacrifice, not about a god choosing to manifest himself in a gory way.

For this to happen, she would have to live past the Emerald Grove questline by letting her slaughter the tiefling refugees and druids inhabiting the grove. And if this happens without killing Halsin in the goblin camp beforehand, he will eventually hunt you down at your camp in retribution for the massacre at the grove with the game forcing you to kill him. Furthermore, she's also one to Wyll and Karlach if you side with the Goblins in Act 1 against the Grove. However, if you simply avoid taking a side and move on to Act 2, she can be recruited there while keeping Wyll and Karlach. Halsin will still refuse to join you though. Patch 5 added the ability to recruit her in Act 2 if you knocked her out on Act 1. However for this to happen she must have the "Temporarily Hostile" status, which means you must either attack her without talking to her or commit some kind of crime such as stealing, or attacking others, in front of her. Halsin will travel alongside you to your camp but won't join the party due to her presence. If you successfully knock her out like this, she'll be programmed to recover after the next Long Rest, incentivizing you to deal with the rest of the goblin leadership before resting at camp.]]
