Actually, no. From a flashback we can see that Hoenheim actually looked very similar to how he looks currently. Now of course, this raises the other question of why does current Hoenheim look like past Hoenheim. Did he pick a similar looking body or does the body change to resemble the original over time?

* OurHomunculiAreDifferent: Homunculi here have a different origin than that of the manga, being [[spoiler: the artificial body created when a person attempts human transmutation for the purposes of resurrection]]. They start off as deformed, miserable beings, but after [[spoiler: Dante feeds them fragments of Philosopher's Stones]], they grown into humanoid forms with unique powers and a strong HealingFactor that makes them nearly immortal.
** In addition, the [[spoiler: artificial Ninas created by Tucker]] are even closer to more usual depictions of homunculi, being created inside large test tubes. Just like the actual Homunculi, they are said to be soulless.
