Not always. I just realised an instance of this trope applying to a scene in a fanfic: one character was experiencing power incontinence, which resulted in said character exploding (granted, this character could heal herself from these kinds of injuries, so she lived through it...which I guess makes it less horrific). It later occured to me what the instance could relate to....whether the author meant for there to be a narm-soked pun is beyond me, but in this case, it had nothing to do with sexual innuendos.
