The Clowns are made up of ghouls that were raised in Donato's Orphanage. It was implied he was feeding his victims to the rest of the orphans, it could have been because most if not all were ghouls. Donato was locked away for more than 10 years and the Clown members we have been introduced to (Uta, Itori, Roma, and Nico) likely would have been teenagers when he was captured.
