Rena from Higurashi and Mitsuko from Battle Royale, which is saying something since everybody in both those series were having mental breakdowns. And definitely Mikami from death note.

Kamisama Dolls provides lots of fodder for this thread, given how the titular thingies seem to induce brain damage to their wielders. Special mention to Kuuko, who is just a random civilian but makes a good bid to out-crazy everyone else in the series (although it's hard to top Mahiru).

Dilandau from Escaflowne. He starts obsessing over a LITTLE scratch he gets when the protagonist hits him in the face, and when his lower rank officer asks him not to scratch at it, he snaps at him. He loves fire and destruction, and laughes his butt off when cities and people are screaming and dying in large numbers. He also can't stand seeing his prey escape or not getting his way with people.

Next would be Harry Mcdougall from Outlaw Star. He becomes obsessed with a female in the series, and tries to ask her out. She refuses, at which point he snaps and starts threatening her, and tells her he'll kill her. He later dies in a Heroic Sacrifice moment.
