In practical terms, it's usually regarded as Roger being the one and only person to reach Raftel, the final island on the Grand Line. Reach that and match Roger's achievement, and you can find yourself Pirate King as a result.
What I really liked about the previous revelations - not only did we never see it coming, but in hindsight, we should have realized something was up. We've known for years that Sanji is from the North Blue and how the four seas are divided up, but never put together that that's a hell of a trip for a little kid to take. And why was he on that ship Zeff attacked, anyway? Where was he going?
It reminds me of that moment we are introduced to Luffy's brother and realize how very little we know about our main character. Seriously, for a while there Luffy's background was one of the biggest mysteries in the series.
