They both sound very similar, where a prominent character fades into the background.

Out of Focus is a brief, temporary change where a character is in the background, typically when someone else has A Day in the Limelight. Demoted to Extra is long lasting, just one step above Put on a Bus because that's how minimized their relevance became.

Demoted to Extra compares an adaptation [edit: or sequel, thank you] to the original, not different parts of the same work.

[up] Wait, really? If that's true, then it needs a new image, since it depicts movie sequels rather than adaptations.

Demoted to Extra means that a major or significant character has been downgraded into a background or minor character. Out of Focus means that a certain character gets less focus because of a certain other character or that character lacks significance overall and gets less screen time then they are billed for.
