I have a feeling just writing "because the author didn't think of it" is against the spirit of Headscratchers, but at some point, that's just the answer. There are inconsistencies big multi-author book and video game series. There just are, and they never mean anything, they're just a later author writing something without considering what earlier authors wrote. So where's the line? When is it OK to post "because it's a work of fiction written by fallible human beings"?
