I think 20.1 and 20.3 together would work better than just 20.3 because then you can make out the lost arm and collar more easily, plus the pain she's going through.

I think 20.3 works well enough alone since people generally have two arms, so you don't need a previous picture to know that she's missing one arm. My vote goes to 20.3
