To be honest I can't tell from the teaser whatsoever. The first thing that comes to mind is some sort of mystery game where you have to find out where the hell you and that child came from but that's probably just me projecting since the scene was so bizarre and without any context.
It has a lot of horrific, generally fucked-up iconography, at least. I'm assuming a horror-focused game from that trailer, whatever on Earth the mechanics of it might be.
It definitely reads like horror to me. Except for Norman Reedus's ass, which wasn't horrible.

But it's also packed with symbolism, which is something to be expected when Hideo Kojima is finally able to do exactly what he wants for the first time in 20 years.
Like, it's not hard to think that the five tags on the chain around his neck, and the five figures in the sky are Metal Gear Solid 1 through 5, and letting go of the past may be a theme of the game.
Btw, in the end, correct me if I'm mistaken, are the dead whales still connected by the umbilical cord? What could that mean?

In a time where most trailers leave me bored and apathetic, this one made me feel confused and uncomfortable (and maybe just a little bit aroused)
So it's got that going for it. Definetly going to keep this one on my radar.
Even if the teaser has nothing to do with the actual game, the idea of waking up on the ocean floor surounded by dead marine animals is incredibly awesome.

The name, nudity, and general sense of tragedy make me wonder if this takes place in some sort of afterlife situation akin to Riverworld, but with less famous historical figures.
Was thinking that too. Looks like it's going to have an environmental/cyberpunk horror theme; it would explain why the sand is oily and black, as if the animals were killed by an oil spill, and the odd cables connecting both Norman Reedus's character and the infant. Basically, it might be his AI's 'conscious' feeling guilt over some transgression.
