Ah. Yes. I would not worry overly, though; if her Soul Gem were truly lost, neither Madoka nor the Laws of this world would allow such a state to continue. Far likelier the Soul Gem was broken in the battle, or became corrupt, in which case either the Law of the Cycle or the Law of this world would have revived her.
The worst case that I can think of... would be if her Soul Gem became fully corrupted, and the Laws of this world prevented it from being retrieved by those of the Cycle. In which case... she would become a suffering monster who brings pain and misery to all those around her. But from what you've told me... there is no sign of such a monster.

Isana raises an eyebrow. Save her? From how it sounds, it seems this encounter was a while ago... whatever effects it had have likely been long played out. It would not surprise me if Isaki were right now observing this moment from the Sanctuary, and cursing Madoka's wards or the weak dimensional connection for the fact that she cannot return to reassure you.
-to Observer- But... we have different perspectives, knowledge, and abilities. We may be able to find a solution which is far easier, or may not even be possible or comprehensible, for you...
