Past Patrick is wearing a sweater, current memory keeper Patrick is not. He said previously that his mother is "beyond the barrier" (issue 7, page 64) when Alison asked who she was, which accounted for her silhouette and lack of displayed memories.
The most obvious interpretation of "not anymore, thankfully" is that memory keeper Patrick is glad he does not have to re-experience reading her mind when he returns to this memory, because he's locked that part of the memory away beyond the oldest memory barrier. This line serves as exposition and draws attention to the fact that he is not recalling her thoughts.

Yeah I think killing was pretty much implied, with the "make it painless" line.
"But than I made a barrier and put my need for love behind it and when I got rid of my need for love the lack of it didn't sting so bad"

More psychic self-surgery. This is fascinating. My current theory on this arc is that Patrick's seeming insanity on arrival is due to all of these bits he's locked away coming back in full force. However, I don't think it's due to his power, but rather due to him honestly wanting to become more human for Alison.
And Patrick's rise to Menace, with confirmation of what we've long suspected, that he was an unusually early trigger for some reason.
I'm sure we won't see what's behind that door before the end of this arc.

There was another strip before this one on Tuesday but all it really does, in my opinion, is set up that all of Patrick's mental constructs follow the instruction of that poor broken little boy.

This kind of makes me think... what was the purpose of Kid Patrick taking her through that? Did he hope she would understand? Was he undecided until then that she had to die? Did he have to gather forces? Did he never really want to kill her, and expected she'd escape?

Eyeh, but that was a lot of rigmarole, taking her through the painful genesis of Menace, before springing the trap. He's playing some sort of a game here, but I'm not sure what.

As far as I can tell "Not anymore" is said by the mental keeper version rather than by the original one in the memory - he's sitting on the opposite side. He isn't reading her thoughts because he sealed all memories of it off.
