Blaz Blue has my favorite game over theme.
It has a sort of soothing, homely feel to it. It's basically saying "It's over. You're dead. You've fought hard. Now you can finally rest."

I quite like "Savage Sadness" from Chaos Rings.

Deus Ex has a different game over theme for every stage, but in particular I like the Paris theme.
