And that provided the needed distraction. Now it's a stand-off, and you outnumber them. Unless the Navy troops start ganging up on you (most are unarmed and essentially all are too drunk and stoned to fight effectively) RVI team has the upper hand.

Yes, she is. But that's a formality: Akane doesn't get deployed, she stays aboard. Even on the ship, her authority is very limited.
Medics are pretty much autonomous: The Chief is responsible for making sure medics are up during their shifts, and she makes sure the XO is kept up-to-date on supplies requests, that's it. Aside from that, she's a regular medic.
When it comes to protocol, medics refer to the Chief Medical Officer as Chief, and the CMO refers to medics as Dr. + Last Name. Protocol applies only to formal situations: Informally, there are no protocol rules: Some people are on a First-Name Basis, others on a Last-Name Basis, others are referred to by nickname or rank or any other way they can come up with.
