I guess I assumed that the point was that she was communing with her spirit, thus at hyper-speed, and thus a fly moving at normal speed is completely bizarre and indicative of shenanigans.

Looks like Zarei is inadvertently putting her in some kind of arm twist hold.
I don't think so... Zarei's only holding the pen. Either Penny's so determined to not let go that turning the pen resulted in her arm being twisted (maybe it's glued to her hand?) or she's over-reacting to Zarei's comment.

Worth noting: the fly seemed to be exactly where Patchworm was (compare this page and this one) and I don't think we actually know what Patchworm's tool is. It's possible that the fly is Patchworm? Maybe he's like Eightfold and is a relatively weak spirit so he can switch tools frequently without much issue? Maybe he's possessing the fly, and then the fly acts like a tool for Zarei?
My best guess at the moment, anyway.
