The game actually began thousands of years ago, but you didn't realize it until you sat at the dealer's table. You play the game by recalling your own past lives, whether they succeeded in their quests or failed. The dealer even tells you that from his perspective, the game is already predetermined. When you lose, there are seemingly no consequences, but in fact it is possible that in that lifetime, you suffered a horrible death and that hundreds of years have passed until you reincarnated again. Whoever defeats the dealer replaces him. The real mindscrew is, he's your future self and he's trying to guide you to step into his shoes.

As the fallen god on the path to [[spoiler:the gateway to the past]] is kind enough to point out by the time of the second game their are only six true gods left in the world. The dealer based on their abilities and behavior appears to be at least one of them and specifically a god of change who exists to bring about new events in the world. Additionally he holds some mutual power with The Fates, they allow him to 'pull strings' and run a potential replacement through a gauntlet repeatedly to determine their worthiness. The whole goal is to make the changes to the world he can then find a replacement worthy to take over whose temperament and behavior will shape the next round of changes. [[spoiler:Kallus the previous replacement]] was just so terrible it necessitated the previous Dealer stepping in to correct it.

It seems strange that it would be the last trial after a long quest, wouldn't it ? Sure, you can lose a session, but you can go right back at it without any penalties, and you even keep what you unlocked after a failed run. However, it's only how it feels from our perspective. Maybe the Adventurer actually feels every wound, and therefore every deaths, he suffers during a session, which would get pretty unbearable. And even without that, the cards are made from his memories... Meaning he has to relive all his lies, all his failures, all his shortcomings. And considering they can have different outcomes, maybe the Adventurer sees what could have happened had he decided to act differently. This is why the Dealer says no one won the Game so far : no one was strong enough mentally for facing the errors of their way and the consequences of their sins.
