Alejandro, Amy, B, Beardo, Beth, Bridgette, Dawn, DJ, Geoff, Jo, Lightning, Sammy, Scarlett, Shawn, Sierra, and Topher compete this season, while Sam and Dakota host the Aftermath.

The campers compete in dangerous challenges while avoiding the island's mutated wildlife. One camper gets involved in a conflict with the mutant shark named Fang, two campers express interest in each other, and three campers look to take the competition by storm. Contestants from the first three seasons also appear in cameos.

There's a catch to winning though, one member of the winning team must serve exile on Boney Island, where an immunity idol is hidden. Older competitors and newer competitors collide, new friendships and rivalries form, and there are unexpected twists never seen before.
