[[spoiler:The innermost circle is a reclusive cult in the mountains that seeks enlightenment in the Void, and is a successor to the original cult that first created the Outsider.]]
19* EqualOpportunityEvil: The Eyeless' membership includes individuals from across Karnaca's social classes, and their operations range from back-alley fight clubs to upscale dens of vice.
20* TheHeavy: While the main target for Billie and Daud is The Outsider blaming him for all the chaos and destruction committed by his black magic, the latter seems quite indifferent on whether or not they do succeed in killing him. The Eyeless, meanwhile, are a powerful cult that has ties to the Outsider's history and possesses the Twin-bladed Knife needed to kill him, so they serve as the main opposition for Billie and Daud during the story.

The vile concoction is alleged to grant visions of the void, since rats are associated with the Outsider. The drink has a horrible stench, however, and the player can use bottles of plagued spirit as improvised stink bombs, stunning enemies as they gag and choke.
