However, Kelly's image and legacy are marred by numerous allegations of sexual abuse and misconduct, many involving minors. These accusations, dating back to the early stages of his career, [[OvershadowedByControversy cast a shadow over his accomplishments]]. His controversial relationship and marriage with then-underage singer Music/{{Aaliyah}} in 1995 and allegations of abuse by his ex-wife Andrea Kelly further tarnished his image. In 2017, a lengthy story from [[Website/BuzzFeed BuzzFeed News]] accused of Kelly allegedly running a "sex cult" and engaging in human trafficking of women (some lured in as teenagers) with the promise of helping them establish singing careers. Despite these claims, his career remained largely unaffected until the release of the Creator/{{Lifetime}} documentary series ''Surviving R. Kelly'' in 2019, which sparked a nationwide reckoning with his actions. In 2021, he was convicted of sex trafficking and other charges, resulting in a 30-year prison sentence, and convicted in a second trial in 2022 of three of 13 counts of producing child pornography. He also has lost a civil trial against one of his accusers and was found liable in a default judgement against another. Kelly continues to challenge his convictions and maintains his innocence.

However, Kelly's image and legacy are inseparable from the numerous allegations of sexual abuse and sexual misconduct made against him, a number of them involving minors as victims. Initial reports along these lines date back to the early years of his career, and they would eventually grow to [[OvershadowedByControversy overshadow his career]].

His relationship with an underage Music/{{Aaliyah}} in the early '90s earned strong scrutiny, especially after it was revealed in 1995 that the two were briefly married when Aaliyah was 15 before her parents had it annulled; it was revealed in later years that Aaliyah considered the relationship abusive. His ex-wife Andrea Kelly also alleged he was abusive during their marriage, successfully obtaining a restraining order against him. Although Kelly was found not guilty of all charges in that case, it didn't help his reputation much.

Multiple other women claimed to have had romantic or sexual relationships with Kelly when they were still minors. In 2017, a lengthy story from [[Website/BuzzFeed BuzzFeed News]] accused Kelly of human trafficking, stating that he was keeping multiple women captive in an abusive "sex cult", luring them in as teenagers with the promise of helping them establish singing careers. The allegations never negatively affected Kelly's career or public image in any significant way, much to the dismay of his accusers, until the 2019 premiere of the Creator/{{Lifetime}} documentary series ''Surviving R. Kelly''. The documentary re-litigated his abusive relationship with Aaliyah, his trial on child pornography charges and the numerous allegations against him; it subsequently led to a nationwide reckoning with Kelly and renewed discussions of the abuse allegations, especially how they had been ignored for so long.

On September 27, 2021, R. Kelly was formally convicted of sex trafficking, sexual abuse of a minor and racketeering in federal court. He was sentenced to 30 years in prison on June 29, 2022, with a second trial in September for producing child pornography of the same year ending with Kelly being convicted of three of the 13 counts of producing child pornography, though Kelly and his co-conspirators were acquitted of trial fixing charges in a 2008 state trial. He also has lost a civil trial against one of his accusers and was found liable in a default judgement against another. He is still challenging his convictions to date.
