Furry Kef: I wanted to add this:
Non-school example: NBA Jam: Tournament Edition was ported to consoles while NBA players were trading teams. The programmers moved them to the appropriate teams, but they didn't update the graphics, so several player portraits show the wrong uniform. (In game, they do wear the correct uniforms, of course.)
...I decided against adding it, though, since it doesn't quite fit the trope. The trope has more to do with what the wrong uniform says about their personality or how they fit in (or fail to) with the rest of the cast; in this case, the difference in uniforms is more incidental.
