Because the victim is surprised by a "pineapple" grenade. That's what those grooved ones are called.

Perhaps an alternative set of names, in curiosity, can be;
- Hawaiian Surprise?
- Hawaiian Gift?
- Hawaiian Birthday?
- Hot Potato Residue?
- Egg Out A Microwave?
- from 炸蛋 "fried egg (actual: 煎蛋)", a comic-license mandarin homophone from the word "bomb" (炸彈), also a possible way to describe accidents of eggs exploding after being cooked in microwaves
I like the trope name and all, but I'm just curious, where does it come from? Why is it called Pineapple Surprise?
