For The Vampire Diaries in the books it's not fan speculation at all, Katherine is quite explicit at waning all 3 of them to be Happy together, it's the brother who won't have it and kill each other. Elena considers Kathrine a naive idiot for this.

- Actually, there's a scene in the manga in which Orihime openly wonders what would happen if both she and Rukia liked Ichigo◊, meaning that she might be open to a Rukia/Ichigo/Orihime trio with Ichigo as the vortex. Considering that Orihime loves Ichigo but also adores Rukia and they both care for her whether as a friend or more... Hmmmm!
I Can't believe that nobody mentioned Superman/Wonder woman/Batman as the ultimate threesome in the comic books section. >= Hell, it was even kind of played with in the JLA series with Wonder woman having shown a thing for the two men. The two men had shown signs that they'd be intersted in her, only if it wasn't for other things, like Superman having lois lane for a girlfriend. Plus there's been a tiny bit of ho yay between Superman and Batman at times, So you just know that if it wasn't for other factors, All three people wouldn't mind hooking up. And plus they all have worked very well together as an power trio.
