The matter with Itami ignoring Kuribayashi in ep. 4 is a translation issue. It's not that the girls were calling him Captain while he's a Lieutenant, it is more to show that he's more into his web novel updates rather than his squad people. They were calling him "taicho" in Japanese (which is precisely a "squad leader"), rather than "Captain", which would've been "san'i" in modern JSDF. It's just that the translators chose to use a wrong term for some reason.
