I think several of the examples don't fit the bill. DIAA isn't "there's no good outcome" but "people don't care" - the hint lies in the apathy. So if, as an example, people avoid playing a certain mission in Fallout 3, that's not apathy. That's actually caring a lot. You don't want people to die. DIAA would be "yeah, whatever, somebody will die here, let's get over with it".
