A propeller plane crashes. Investigators discover that the propeller angle on one side was dangerously low, and further discover that it had detached from its control mechanism. The manufacturer says this is impossible, as if the propeller gets disconnected it should reach a high angle which renders the propeller useless but perfectly safe, and the plane should be able to be landed safely with just the other propeller. Tests in the lab agree with the manufacturer; however, when the investigators actually get a test plane up in the air, with an extra failsafe to prevent the propeller angle from becoming dangerous, the propeller ends up going to its lowest permitted angle when the plane is preparing to land (which matches the original crash).

The result is that the plane crashed with no survivors.

I'm not American or British, so I don't know how much Truth in Television is there to this type of character:
A group of artsy young adults, usually with a lame Improv theater vibe, who'll go to a middle school to give some sort of special presentation on the dangers of bullying, drugs, tobacco and so on. Of course, their speech is so chock full of Totally Radical slang and poor jokes, the kids are more likely to mock them than to take their lesson at face value.
And so, their presentation often ends up having the opposite effect. If the people who are most deserving of bullying are the ones doing anti-bullying presentations, kids might come out of it already looking for targets. Or, as in the South Park case, the lamest young men in the world assure that kids will grow up to be as cool as them if they never smoke... followed by a Gilligan Cut of the kids trying cigarettes.
Other versions include summer camp counselors, or religious abstinence-only educators who desperately try and convince kids not having sex can be cool and radical.

The mother wants to go to her daughter's recital, but work constraints prevent her from doing this. She does show up at her son's big game, but is called away in the middle of the game to work.

A group of exhausted, usually beaten-up, protagonists taking a moment after a grueling fight to eat and rest. Usually they look worse for wear while the waitstaff looks on confused while taking their orders.
Best example I can think of is the "The Stinger" scene from The Avengers (2012)
