It can also fall under Not Now, Kiddo and Not Worth Killing, if the villain dismisses them as a threat and ignores them.
I wouldn't mind failure so much, if I didn't fail so much.
The hero is fighting the villain, or the dragon, or maybe even just a particularly tough mook, the hero lands a blow that should be fatal, or even just debilitating, but all his opponent does is laugh at the injury like its a joke.
