It's a book about people who give and get heart transplants, inspired by stories about people claiming they dream about their organ donors' lives or whatever, especially if their donor is dead.
There was a chapter about one of the main characters (I think she donated her heart after she died) celebrating her birthday as a young girl. At night, when she and her parents are having their birthday dinner and are about to blow out the birthday cake candles, they ask her what her birthday wish is. She says she wants a baby sister/brother, which sets off the mom (I guess because she was raped or something and the daughter is a product of that, I don't know but it seems like it). She angrily tells her that it won't be happening, throws away the cake, and sends her to bed early while her husband futilely tries to calm her down and point out that the daughter didn't mean anything by it. The next morning, the daughter sees the cake in the trash but there‘ a new cake on the table or the mom is making a new cake to try to make up for her outburst, but I think the chapter ends noting that the damage is already done.
